	Families in Transition-Concord Transitional Housing Est. 2004 Provides transitional housing for 8 homeless single women with 16 children and 2 homeless single women Intensive wrap-around services for up to 12 months with two 6 month extensions possible, includes on site supportive services for as long as needed Eight-2 bedroom apartments and two-one bedroom apartments
